NEJM Images: More Than Asthma

A 26-yo man with a long history of poorly controlled asthma and severe environmental allergies was admitted for an exacerbation of asthma. A chest CT showed bronchiectasis along with mediastinal and hilar lymphadenopathy.

What was the reason?
Allergic Bronchopulmonary Aspergillosis.
